General Information
Certified copies of Missouri birth and death certificates are provided by Vital Records in the Administration Office of the Scotland County Health Department, Monday-Friday, from 8:00 am to 4:00 pm.
Application for Birth or Death Certificate
Birth Certificates
We issue certified copies of birth certificates for $15.00 each.
Requirements:
- Born in the State of Missouri
- Births from 1920 to present (Births prior to 1920 are available through the Bureau of Vital Records in Jefferson City, MO, 573-751-6378.)
- Identification – one primary document with your name and photo such as driver’s license, U.S. military card, U.S. passport, work or school ID OR at least two alternate identification documents which contain your name and the name of the company or organization
issuing the document.
Death Certificates
We issue certified copies of death certificates for $13.00 for the first copy and $10.00 for each copy thereafter that is purchased at the same time.
Requirements:
- Died in the state of Missouri
- Deaths from 1980 to present. (Deaths prior to 1980 are available through the Bureau of Vital Records in Jefferson City, MO, 573-751-6378.)
- Identification – one primary document with your name and photo such as driver’s license, U.S. military card, U.S. passport, work or school ID OR at least two alternate identification documents which contain your name and the name of the company or organization
issuing the document.
Records Outside of State
Certificates for births, deaths, marriages and divorces that took place outside the state of Missouri can be obtained by visiting the National Center for Health Statistics.
